I/We hereby declare, acknowledge, and confirm that:

1. All the particulars and information and details provided by me/us in this Application are true, correct, complete and up-to date in all respects and that I/we has not withheld any information whatsoever.
2. I/we shall furnish such additional documents or information as may be required by ICICI Bank, from time to time, in connection with the Facility.
3. ICICI Bank shall have the absolute discretion, without assigning any reasons to reject this Application and that ICICI Bank shall not be responsible/liable in any manner whatsoever for such rejection or any delay in notifying me/us of such rejection and any costs, losses, damages or expenses, or other consequences, caused by reason of such rejection, or any delay in notifying me/us of such rejection, of this Application.
4. A non-refundable processing fee shall be payable for processing this Application.
5. I/we have read and understood the applicable interest/charges/fees available on ICICI Bank website https://www.icici.bank.in/business-banking/business-loans/schedule-of-charges and the details of the same shall be communicated to me/us through credit arrangement letter.
6. ICICI Bank /its group companies have the right to retain the photographs and documents submitted by me/us with this Application.
7. I/we unconditionally and irrevocably authorise ICICI Bank to deactivate any existing debit card issued to me/us by ICICI Bank, upon disbursement of the Facility in the Current Account.
8. I/we have read and understood ICICI Bank's 'Privacy Commitment' available on https://www.icici.bank.in/privacy and acknowledges that the same shall be subject to changes by ICICI Bank from time to time at ICICI Bank's sole discretion and I/we shall keep myself/ourself updated with the same.
9. I/we authorise/ give my /our consent to ICICI Bank to classify me/us as per the categories of Priority Sector Lending (PSL) based on the Udyam Registration Certificate (URC).
10. I/we expressly and unconditionally authorize ICICI Bank to, disclose, transfer or part with any information (including sensitive personal information, location etc.) or any other information related to me/us when ICICI Bank considers such disclosure as necessary, with:
a. Auditors, credit rating agencies/credit bureaus, statutory/regulatory authorities, governmental/administrative authorities, Central Know Your Customer (C-KYC) registry or SEBI Know Your Client registration agency, having jurisdiction over ICICI Bank;
b. Agents of ICICI Bank in any jurisdiction; service providers, professional advisors, consultants or such person with whom ICICI Bank contracts or proposes to contract. (Collectively referred to as �Permitted Persons�).

For the purpose of:

a. Provision of the Facility, completion of on-boarding formalities and servicing and renewal/enhancement of the Facility; or
b. Complying with KYC / Applicant's due diligence requirements, anti-money laundering checks; or
c. Compliance with applicable laws or any order (judicial or otherwise), statutory/regulatory/legal requirement, including disclosure to information utilities; or
d. Credit monitoring and review of Facility availed; or
e. Authentication or verification; or
f. Research or analysis, credit reporting & scoring, risk management, participation in any service-related communication; or
g. Electronic clearing network and for use or processing of the said information/data; or
h. Disclosing any default in payment; or
i. Recovering the Facility including all interest and other charges

11. I/we understand that I/we may proactively opt-out of this consent at any time or update my/our preferences by visiting ICICI Bank branches or by calling ICICI Bank Customer Care Number 1800 1080.
